Holigenix Healthcare is a HIPAA-covered pediatric private duty nursing agency. We collect personal information from referral partners and families through this website only to coordinate care and respond to inquiries. We never sell your information.
What we collect
- Information you submit through our contact / referral form (name, email, phone, organization, message, and whether you checked either optional SMS consent box).
- For partner referrals: limited de-identified context about the patient (initials, age range, service type, urgency, insurance category).
- Standard server logs (IP address, user agent, page accessed) used for security and performance.
How we use it
- Respond to your inquiry and coordinate care intake.
- Maintain a record of communications in our HIPAA-compliant CRM (GoHighLevel) under appropriate access controls.
- Improve our services and the operation of this website.
Protected Health Information (PHI)
Please do not submit Protected Health Information (full name + diagnosis combinations, medical record numbers, dates of birth, or other PHI) through the website form. For clinical handoffs and PHI exchange, contact our intake team at 1 (888) 857-8667 or use a secure channel arranged with your discharge planner.
Text messaging (SMS)
If you give us your mobile number and check an SMS consent box on our website, or sign our SMS consent on an intake or onboarding form, Holigenix Healthcare may text you about nurse scheduling, visit reminders, care coordination and, if you chose it, agency news and events. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. Information sharing with subcontractors in support services, such as customer service and our messaging platform provider, is permitted. All other categories exclude text messaging originator opt-in data and consent.
